Tri-County Health Department issues legal, certified
birth and death certificates
Certified birth and death certificates are legal documents that are printed on forgery-resistant
safety paper with watermarks, raised engraving, background
security patterns and a unique certificate identification number.
Please note that hospital baby footprint certificates cannot be used as a legal document.

In-person payments can be made by cash, check, money order or credit cards, including MasterCard, Visa or Discover cards. American Express is not accepted.

The administration office is a 3-story brown brick building one block east of I-25, and between Orchard Road and Arapahoe Road. It backs up to Yosemite Street and is located 1 block north of the Arapahoe Light Rail Station Garage on the east side of I-25. (Map)

Certified Birth
Certificate Fees:
We can issue certified birth certificates for anyone born in any Colorado county.
Fees are
$17.75 first copy,
and $10 each for each additional copy of same name if ordered at the same time.
Fax orders: $10 credit card fee will be added. Expedited FedEx shipping is available for mail orders for an additional
$20.
Certified Death
Certificates and Fees:
We can only issue certified death certificates for deaths that occurred any year in Adams, Arapahoe and
Douglas Counties.
Copies can also be ordered through the funeral home making arrangements. Death certificates for a veteran of the armed forces can receive one free Veterans Administration office copy.
Fees are $20 for the first copy, $13 for each additional copy of the same name if ordered at the same time. Expedited FedEx shipping is available for mail orders for an additional
$20.

Who Can Order:
• The person named on the birth record
• Members of the immediate family (Proof of relationship is required)
• Legal representatives of those named above
• Others demonstrating a direct and tangible interest in the record
Required Information:
•The person's full name as it appears on the birth record
• Date and place of birth
• Parent's names along with mother's full maiden name
• Purpose for the copy
• Applicant's relationship to the person on the certificate
• Applicant's signature
• Payment for the number of copies requested
• Photocopy of the requestor's valid identification
