Peak Wellness - Health Screenings for Men and Women

Peak Wellness

by taking care of yourself"


Peak Wellness provides low-income, uninsured and under-insured people ages 40-64 with chronic disease risk factor screening and referrals, in an effort to prevent cancer, diabetes, heart disease and stroke.

Health Screening
Services:
Free cholesterol screening, blood pressure readings and blood sugar tests to qualifying individuals. Women also receive free breast exams, mammograms, Pap tests and pelvic exams. Weekly clinics are scheduled across the metro area.

Peak Wellness
participants receive one-on-one personalized sessions with a nurse and/or registered dietitian to help them change their risk factors and improve their health. Participants who need additional medical follow-up or mammograms are referred to community clinics in partnership with us.

Peak Wellness services are available in our Aurora, Castle Rock, Commerce City Englewood and Northglenn offices. (Maps)

In order to qualify for these services:
• A person must be 40-64 years old
Be legally present in the United States
• Live anywhere in Colorado
Meet income guidelines
Have no health insurance or not be able to afford the deductible for insurance
Women should not have had a mammogram in the last 11 months
