HCP-A Program for Children and Youth with Special Health Care Needs
HCP is a program that offers support and resources for parents and their child with special health care needs, ages birth to 21.
The HCP team believes that all families deserve the opportunities and support in order to promote the maximum potential for their children, and HCP can help with information and resources on: health care, child development, nutrition, education, recreation, insurance and support groups.
HCP support can lead to a more successful and fulfilling experience for everyone - at home, at school and in the community - and help prepare a child for a meaningful adult life. Services are provided free of charge to families in Adams, Arapahoe, Douglas Counties.
Dear valued community-
Thanks for being an important community partner for the Tri-County Health Department HCP program, a program for children and youth with special health care needs. Tri-County Health Department (TCHD) will be dissolved December 31, 2022 and in anticipation of this, the HCP program at TCHD is sunsetting. Beginning October 1, 2022, HCP will not enroll new families for assessment, collaborative care plans, or ongoing care coordination. Families previously enrolled will continue to receive services as needed and the team will continue to be available for families that need information, resources and community connections until mid-December.
The HCP Team will continue to support the children and families from October 1 to December 15, 2022 by:
Providing information, resources and community connections to organizations that offer medical, behavioral, and mental health services,
Linking families to resources for food, transportation, financial/employment, health insurance, rent/housing/utilities, child care/respite, primary and behavioral health care, medication, medical equipment, and dental care, and
Working with you as our community partner to explore care coordination options for families in need.
