Business Checklist: Guidance for Developing a COVID-19 Plan

  1. ONLY businesses that have been referred to this form by the Business Recovery Task Force need to fill out this form and click “submit” below.

  2. If you are not sure that you need to fill in this form, please e-mail covidbusinessrecovery@tchd.org.

  3. Once this form is completed, the following checklists and descriptions will serve as the reopening plan for your business. The items listed in each checklist are required to be in your COVID-19 Business Reopening Plan. To move forward with reopening, you will need to make sure you have planned for each item below. If a particular item does not apply to your industry, please describe why in the description.
